Physician’s Statement You may need to provide a doctor’s note for time missed or for work restrictions as a result of a workplace illness or injury. Venable may also require a physician’s note releasing you for work or from work restrictions. This note is subject to verification by Venable’s physician at our expense, if we so choose. Please remember, in order to help prevent accidents, please report immediately to your supervisor any work condition that you believe is unsafe or unhealthy.
