Eligibility You are eligible for short-term disability benefits if you are:
• A regular full-time employee or regular part-time employee (scheduled to work 25 or more hours per week) are eligible as of the first of the month following date of hire. Past service credited for employees returning to the Firm’s employment does not apply to short-term disability benefits. AND • Unable to work because of an illness, including sickness, disease, other defined medical condition, pregnancy or complications from pregnancy, or unable to work because of an injury, AND • Under the appropriate treatment and care of a physician To receive benefits, you must file a claim for short-term disability benefits and be approved through our short-term disability claims administrator. Claims must be filed no later than 60 days after your first day of disability. You are advised to submit your claim as soon as possible in order to avoid a lapse in salary. You may be required to be
